Couples therapy costs and insurance: questions before booking

By RelationshipInfo · Updated September 19, 2026 · Sources checked 19 September 2026

The cost of couples therapy depends on the clinician, session length, format and location. Compare the complete course you might use, not just the advertised session fee. Insurance coverage for mental-health treatment does not by itself confirm payment for relationship counselling; check the exact service with the insurer and provider.

Current published fee examples

Published provider examples checked 19 September 2026; reconfirm before booking.
Service and location Published example What the figure excludes or leaves open
Tavistock Relationships — London / online Qualified therapists: £100–£200 per hour; trainee options are also described. Check the initial consultation charge, clinician and booked duration. This is one provider’s range, not a UK average.
NSU Health — Fort Lauderdale–Davie, Florida Student practice: US$25 per counselling session. This is the supervised student service. Faculty practice has separate payment arrangements; ask about session length and eligibility.
Ackerman — New York Insurance and needs-based sliding-scale options; no single self-pay figure stated on the clinic page. Ask the intake team for your proposed fee and confirm coverage for the actual service. A listed insurer does not establish your personal benefit.

These examples show different service models, not a price ranking. Currency, clinician experience, supervision, location and session length affect the comparison. None is a market-wide average, a guaranteed place or a quote for your care.

Build a budget from a written quote

Ask for the initial assessment fee, ordinary session fee and duration, any longer-session price, cancellation charges and charges for reports or additional contact. Check whether a price applies to the couple or to each person. Include travel, childcare, time away from work and exchange-rate costs when relevant.

For a planning example, eight one-hour appointments at the published Tavistock qualified-therapist range would total £800–£1,600 before any separate assessment or other costs. Eight sessions at NSU’s published student-practice fee would total US$200. The number eight is an illustration, not a recommended treatment length; these services also differ in setting and staffing. Obtain the actual duration, initial fee and proposed review point before calculating your own budget.

Ask the insurer about the exact service

HealthCare.gov explains that US Marketplace plans include mental-health and substance-use benefits, while specific benefits depend on the plan and state. This does not establish that your proposed couples service is covered. Ask about the clinician, the billed service, network status, deductible, authorisation and any limits. Check the Marketplace coverage guidance.

Request an explanation you can keep and ask the provider what happens if a claim is refused. Do not ask a clinician to use an inaccurate diagnosis or billing description to obtain reimbursement. Discuss whose clinical record is involved and what information may be shared for a claim.

Lower-cost routes to investigate

Ask local services about reduced-fee appointments, supervised training clinics, charitable provision or employer benefits. Availability and eligibility vary; a directory listing is not confirmation of a subsidised place. Check who will deliver treatment and what supervision is offered.

A workshop or self-guided course may cost less but provides a different service. It may not include individual assessment, confidentiality arrangements for joint work or help with complex clinical needs. Compare what is included rather than treating products as interchangeable.

Before paying for an intensive or retreat

Obtain the programme, cancellation terms, accommodation arrangements and follow-up plan in writing. Separate clinical sessions from hospitality and optional activities. Ask whether the assessment can recommend against attendance and what refund terms then apply.

Large upfront payments, international travel and time away can create pressure to continue despite poor fit. Agree how either person can stop participation. A high price does not establish stronger evidence, professional regulation or permission to practise where you live.
