Include the page address, the statement that needs attention and, where possible, a reliable source. Reports are saved privately for the website administrator. The form does not request your name or email and does not provide a reply service.
Do not send medical records, intimate details, passwords or identifying information about another person. This form is not monitored as a crisis or clinical service. Use urgent support if anyone may be unsafe.
What happens to the report
The submission is stored as a private WordPress record. An administrator can review, correct or delete it. Reports remain until an administrator removes them; there is no automatic deletion schedule in this release. See the privacy information for details.
